The UCLA PTSD Reaction Index for DSM-5 for Children Age 6 and Younger provides a structure for interviewing parents/caregivers about their child’s trauma history, the full range of posttraumatic stress symptoms, and whether symptoms are related to significant levels of distress, and to problems in relationships with parents and other caregivers, siblings, peers, behavior at school, and development. The Score Sheet provides a method to determine whether criteria for a diagnosis of PTSD and Dissociative Subtype are met.
